PATHOLOGY REFERRAL AND TELEHEALTH CONSENT

1. Purpose

This consent authorises TRT Doctors to collect, use, and disclose your personal and health information (including Medicare number) for the sole purpose of arranging pathology referrals and facilitating telehealth consultations.
This is not a comprehensive medical consultation or diagnostic review. Pathology referrals are provided as a limited telehealth service to assist in the assessment of hormone and related sexual health parameters.

2. Scope of Services

TRT Doctors facilitates telehealth consultations delivered by independent, AHPRA-registered doctors (“Partner Providers”).

Services may include assessment, pathology referrals, and treatment advice relating to Testosterone Replacement Medication (TRM).

TRT Doctors does not dispense, supply, or sell medications and does not operate a pharmacy.

Partner Providers are independent and responsible for their own clinical decisions, prescribing, and record keeping.

If you require urgent care, contact 000 or attend your nearest emergency department.

Pathology referral services specifically include:
• A Partner Provider reviews your details to confirm the referral is clinically appropriate.
• TRT Doctors issues and transmits the referral to you.
• Results are forwarded to you when received from the pathology laboratory.
• You are responsible for follow-up and interpretation of results (normal or abnormal) with your regular GP.

TRT Doctors does not provide emergency care or immediate diagnostic interpretation.

Ongoing management for androgen deficiency or related conditions may only be provided where a patient:
• has undergone a clinical assessment by a Partner Provider;
• is deemed clinically suitable and eligible for treatment;
• has provided informed consent to the proposed care plan; and
• has paid the applicable consultation fees in accordance with TRT Doctors’ fee schedule.

3. Information Collected

To arrange the referral, TRT Doctors collects the following information:
• Full name, date of birth, address, and contact details.
• Medicare number and identifiers (if applicable).
• Relevant medical history, symptoms, and current medications.

4. Use and Disclosure of Information

Your personal and health information may be shared with the following for the purpose of pathology referral and result handling:
• The authorised pathology laboratory (e.g., Clinpath, Sonic, Medway, or others).
• Partner Providers for clinical verification.
• Medicare Australia, if applicable for billing or verification.
• Government bodies where required by law (e.g., AHPRA, public health reporting).

TRT Doctors never sells, shares, or uses your data for marketing or non-clinical purposes.

5. Privacy and Security

All personal information is collected, stored, and managed in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

You may request access to or correction of your records at any time by emailing support@trtdoctors.com.au.

6. Patient Responsibilities

By proceeding with this service, you acknowledge and agree that:

1. TRT Doctors’ role is limited to issuing a pathology referral and providing results.

2. TRT Doctors does not interpret or diagnose pathology results.

3. You must consult your regular GP for review and discussion of all results (normal or abnormal).

4. You will seek immediate medical care for any urgent or abnormal findings or symptoms.

5. You accept that failure to follow up abnormal results may result in health risks.

6. You remain responsible for arranging ongoing care and management.

7. Fees

• Refer to our website for the current referral service fee (currently $49).
• The fee covers administrative and issuing costs of the referral.
• Refunds are not available once the referral is issued.
• Pathology providers may bulk-bill Medicare or charge additional fees if tests are non-rebateable.
• TRT Doctors receives no commission from any pathology provider.

8. Consent and Acknowledgement

By booking and purchasing the referral, you confirm and acknowledge that:
• You have read and understood this consent form.
• You consent to TRT Doctors collecting and using your personal and health information for the purpose of arranging a pathology referral.
• You authorise TRT Doctors to disclose your information to the nominated pathology provider and Medicare (if required).
• You accept responsibility for discussing your results with your regular GP and for any follow-up care required.
• You understand TRT Doctors’ role is administrative and facilitative, not diagnostic or ongoing.
• You understand that if you require urgent medical attention, you must contact 000 or your nearest emergency department.
